Information for Referrers
Collaborative eye care for patients across Melbourne’s north.
Reservoir Optometry welcomes referrals from GPs, pharmacists, allied health providers, schools and local healthcare services for comprehensive eye examinations, urgent eye care and ongoing co-management.
Our approach
Clear communication and practical co-management.
We aim to provide timely assessments, clear written communication where appropriate, and collaborative care pathways for patients requiring ongoing monitoring or ophthalmology referral.
Urgent eye care
Assessment of red eyes, flashes and floaters, sudden visual change, foreign bodies and other acute eye presentations.
Ocular disease
Monitoring and management support for glaucoma, macular degeneration, diabetic eye disease and other ocular conditions.
Children’s vision
Assessment of learning-related vision concerns, binocular vision issues and paediatric eye health.
Technology
Modern imaging and diagnostic equipment.
Our practice uses retinal photography, OCT imaging, corneal topography, visual fields and iCare tonometry to assist in diagnosis, documentation and ongoing monitoring of eye disease.
Clinical findings can be communicated back to referring practitioners where appropriate, including imaging and management recommendations.
Referral pathways
Connected local care.
Where ophthalmology input is required, we can assist with referral pathways to public and private eye services including the Royal Victorian Eye and Ear Hospital.
HealthLink and Oculo integration are currently being implemented to streamline secure communication and referral workflows.
“We believe good referral relationships are built on communication, accessibility and shared patient care.”
