WorkCover Eye Assessment in Reservoir

Work-related eye care in Reservoir

WorkCover eye assessment in Reservoir

If your eye has been injured at work, or you have a vision concern you think may be work-related, contact Reservoir Spectacles to discuss an optometry appointment. John Sweeney is a WorkSafe Victoria-registered optometrist provider.

Tell us what happened, your suburb and whether WorkSafe or its agent has asked for an assessment or report. We can discuss whether we may be able to help, what to bring and the fee before arranging an appointment.

Eye care after a work-related injury

We provide optometry assessments at our Reservoir practice. The assessment is tailored to the concern and to what can be checked within optometry. If more specialised or urgent care is needed, we will explain the appropriate next step.

If a WorkSafe agent, doctor or employer has requested a report, bring the written request or relevant claim correspondence. We can discuss the information requested, what we can provide and any fee before proceeding.

This is an optometry appointment. WorkSafe or its agent decides claim acceptance and whether a particular service or cost is payable.

What to bring

Claim details

Bring your WorkSafe claim number and the name and contact details of your agent or insurer, if available.

Letters or forms

Bring any request for an assessment or report, plus relevant medical or workplace documents you have been asked to provide.

Your usual eye-care information

Bring your current glasses, medicines list and any previous eye reports that relate to the concern.

If you are unsure what information is needed, call us before the appointment.

WorkSafe and payment information

WorkSafe Victoria’s guidance says a medical referral is not required to see an optometrist. Payment still depends on the service being related to a work-related injury or illness and meeting WorkSafe’s requirements. WorkSafe or its agent makes decisions about the claim and payment.

WorkSafe publishes maximum payment rates for optometry services. These rates and item codes can change. We will discuss the applicable service and fee with you; being seen by a registered provider does not guarantee that WorkSafe will pay every cost.

Read the current WorkSafe optometry guidance and optometry fee schedule, or contact your agent if you have a question about cover for your claim.

Serious eye injuries need urgent care

Do not wait for a routine appointment after a chemical splash, a penetrating eye injury, sudden vision loss or a serious injury. Go to an emergency department or call 000 if urgent help is needed. Healthdirect provides first-aid and urgent-care guidance for objects or chemicals in the eye.
